Job Details:

* Provide social work services to hospice patients and family members through the final stages of illness
* Work in home settings including patients' own residences, skilled nursing facilities, assisted living facilities, and adult foster homes
* Deliver intermittent skilled nursing visit support, pain and symptom management assistance, medical social services, and counseling
* Provide spiritual support coordination, hospice aide oversight, and bereavement support services
* Oregon LMSW (Masters-level Social Worker) or CSWA (Clinical Social Work Associate) licensure required
* Oregon LCSW (Licensed Clinical Social Worker) preferred
* Healthcare Provider Level BLS certification required
* Experience as a social worker in a hospital or medical setting preferred
* Previous hospice experience preferred - preference will be given to candidates with this background
* Must use personal automobile for travel - valid driver's license and auto liability/property damage insurance required by law
* Must maintain personal vehicle in good working order
* Serve patients across Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties
