Home Health OT - West Portland OR and Surrounding Areas

Job Details:

* 8-hour day shifts (4x10s and 3x12s cannot be accommodated)
* May visit up to 6 patients per day with a standard of 30 productivity points
* No weekend requirement
* No on-call requirement
* 1 year of skilled home health experience required
* First-time travelers accepted
* Local candidates accepted within a 50-75 mile radius
* Facility may call off up to one shift per pay period (every two weeks)
* Traveler is expected to have their own transportation
* Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
* Attire: Business casual; no jeans; scrubs allowed but not required with no specific color requirement
* Charting: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/OASIS
* OASIS experience required
* Mileage reimbursement at IRS standard rate
* Required certification: BLS
* Oregon license required
* Patient population includes adults and geriatrics with diagnoses such as dementia, Parkinson's, ALS, MS, ortho post-op, and post-stroke
* Skills include but are not limited to: acute care, activities of daily living, cardiac rehabilitation, electrical stimulation, hand injury, hip fracture and total hip replacement, knee injuries and total knee replacement, splinting, and upper extremity prosthetics
