Home Health PT - West Portland OR and Surrounding Areas

Job Details:

* 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
* May visit up to 6 patients per day with 30 productivity points standard (1:1 ratio)
* No weekend requirements
* No on-call requirements
* 1 year of skilled home health experience required
* First-time travelers accepted
* Local candidates accepted within 50-75 mile radius
* Guaranteed hours: facility may call off up to one shift per pay period (every two weeks)
* Traveler must have their own reliable transportation
* Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; system groups clients to minimize drive and windshield time
* Attire: business casual; no jeans; scrubs allowed but not required with no specific color requirement
* Charting: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/OASIS
* OASIS experience required
* Parking: free
* Mileage reimbursement at IRS standard rate
* Required certifications: BLS
* Preferred certifications: Wound Care
* Epic charting experience preferred
* Patient population includes adults and geriatrics
* Patient diagnoses include Dementia, Parkinson's, ALS, MS, Ortho post-op, and Post-Stroke
* Required skills include lifting and transfer, ordering supplies, and scheduling appointments
* Preferred skills include billing, home health, and OASIS documentation
* Oregon license required
