Opportunity:

RN Outpatient Case Manager

Job Details:

* 40 hours per week schedule
* Coordinate inpatients and outpatients in the overall admission process and access to healthcare
* Minimum of 3 years RN Case Management experience required
* Hospital Case Management and Epic experience required
* Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program required
* State licensure as a Registered Nurse (RN) required
* Guaranteed hours do not apply on holidays for clinic workers
* Traveler's permanent address must be at least 75 miles away from the facility
