Job Details:

* Day shift position with a patient ratio of 1:20
* Weekend rotation required every other weekend with holiday availability required; must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day)
* Case load of up to 25 patients daily
* No floating requirement
* Minimum 2 years of acute care case management and discharge planning experience required
* California state license required; pending license accepted but must be licensed upon start date
* Travel experience required; first-time travelers accepted but must be screened
* Epic charting system experience required
* InterQual experience highly preferred
* Business attire or scrubs required
* Computerized documentation required
* Perform admission criteria reviews including ED, Mother Baby, and NICU
* Experience with care coordination, assessments, and discharge planning required
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, patients experiencing homelessness, mental health history or crisis, advance directives, POLST, and protective services process required
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, Medicare, MediCal, and related insurance types required
* Proficient in arranging DME, SNF placement, and ambulance transport independently
* Knowledgeable in Medicare guidelines
* On weekends, may be required to cover up to 2 units with a focus on discharge planning
* Serves patients across all age groups including neonates, infants, adolescents, pediatrics, adults, and geriatrics
* Interdisciplinary support available including IV teams, physical therapy, respiratory services, pharmacy, social services, and more
* Acute behavioral health experience does not meet acute hospital requirements
* RTO restrictions: no more than 7 days
