Opportunity:

RN Case Manager

Job Details:

* Full-time, day shift position
* Serve patients across multiple locations including Bradford, Randolph, Springfield, White River Junction, VT and Lebanon, NH areas
* Screen, evaluate, re-evaluate, and treat patients in a home health setting
* Develop care plans in collaboration with the patient's physician, patient, and family or primary caregivers upon admission
* Periodically review and recertify patient care plans as appropriate
* Coordinate patient care plans through interdisciplinary communication and case conferences
* Provide ongoing assessment of patients' clinical, functional, and emotional status
* Communicate significant changes to the patient's physician and colleagues
* Document evaluations, treatment goals, and objectives
* Teach patients and caregivers self-care techniques as appropriate
* Provide medication, diet, and other instructions as ordered by the physician
* Utilize opportunities for health counseling with patients and caregivers
* Must be a licensed Registered Nurse in Vermont and New Hampshire or hold an unencumbered compact license
* Graduate of an accredited school of nursing required
* Minimum 2 years of experience in home health required
