Opportunity:

Physical Therapist - Home Health

Job Details:

* Restore function to prevent or minimize disability following disease or injury
* Help patients reach maximum performance and function within the limits of their capabilities
* Evaluate and treat patients using medically prescribed physical therapy programs
* Supervise Physical Therapy Assistants as needed
* Perform initial admission and evaluation visits
* Document observations, clinical findings, problems, skilled interventions and treatments, goals, and discharge plans
* Notify assigned Clinical Team Manager of OASIS scores within established timelines
* Travel to patient homes within assigned territory including Atherton, Cupertino, East Palo Alto, Los Altos, Los Altos Hills, Menlo Park, Mountain View, Palo Alto, Portola Valley, Santa Clara, Stanford, Woodside, Sunnyvale, and neighboring cities as needed
* Minimum one year of Home Health experience required
