Opportunity:

Home Health PT - Medford OR and Surrounding Areas

Job Details:

* 8-hour day shifts (4x10 and 3x12 schedules cannot be accommodated)
* May visit up to 6 patients per day with 30 productivity points standard
* 1:1 patient ratio
* No weekend requirement
* No on-call requirement
* Facility may call off up to one shift per pay period (every two weeks)
* 1 year of skilled home health experience required
* First-time travelers not accepted
* Oasis experience required
* Charting system: EPIC/Oasis
* Required certification: BLS/BCLS
* Traveler must have reliable personal transportation
* Service area may occasionally extend up to a 50+ mile radius, though the system works to group clients to minimize drive and windshield time
* Attire: Any color scrubs
* Parking is free
* Mileage reimbursement provided at IRS standard rate
* Relevant skills include acute care, outpatient clinic, rehabilitation, skilled nursing, orthopedic conditions, neurological conditions, post-stroke rehabilitation, traumatic brain injury, home exercise programs, and patient/family education
