Home Health PT - West Portland OR and Surrounding Areas

Job Details:

* 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
* May visit up to 6 patients per day with 30 productivity points standard
* No weekend requirements
* No on-call requirements
* 1 year of skilled home health experience required
* First-time travelers accepted
* Local candidates accepted within a 50-75 mile radius
* Facility may call off up to one shift per pay period (every two weeks)
* Traveler must have their own reliable transportation
* Occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day; system does a good job grouping clients to minimize drive and windshield time
* OASIS experience required
* Charting system: EPIC Remote Client for Home Health (on laptop) and Rover (phone app)/OASIS
* Attire: Business casual; no jeans; scrubs allowed but not required with no specific color requirement
* Mileage reimbursement provided at IRS standard rate
* Parking is free
* Patient population includes adults and geriatrics with diagnoses such as Dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke
* Required certifications: BLS
* Preferred certifications: Wound Care
* Required skills include lifting and transfer, ordering supplies, and scheduling appointments
* Preferred skills include billing, home health, and OASIS documentation
* Epic charting experience preferred
