Opportunity:

Licensed Practical Nurse (LPN) - Hospice

Job Details:

* Full-time position working day shifts
* Conduct home visits under the direct supervision of an RN Case Manager to assist with patient and family care needs
* Assess patient health status and complete nursing interventions in collaboration with interdisciplinary team members
* Communicate changes in patient condition and needed updates to the plan of care to appropriate team members and caregivers
* Assist RN and/or medical provider in performing specialized duties related to end-of-life care
* Observe, document, and report patient physical and emotional needs and reactions to care to the RN
* Educate patients and families in accordance with the care plan established by the interdisciplinary team
* Must hold an active Licensed Practical Nurse license in Vermont and New Hampshire or an unencumbered compact license
* Must have graduated from an approved school of practical nursing
* Experience in medical/surgical nursing and/or home health care required
* Coverage area includes West Lebanon, NH and White River Junction, VT areas spanning more than 70 towns
