Opportunity:
Home Health / Hospice RN
Job Details:
* Monday through Friday schedule, 8:00 AM to 4:30 PM (end time may vary)
* Weekend on-call shifts required
* On-call requirement of 2 times per week, Monday through Thursday
* See 5-6 patients per day depending on type of visit, 25-30 points per week including routine and OASIS visits
* Provide home health and hospice visits for primarily geriatric population with occasional pediatric patients
* Common diagnoses include Heart Disease, CHF, COPD, Pneumonia, Dementia/Alzheimer's, Depression, Stroke, Cancer, Kidney Disease, Diabetes, Liver Disease, and Wounds
* Perform wound care, joint replacement follow-up, surgery recovery, medication management, and catheter management
* Attend weekly IDT meeting and case conference at office, as well as monthly all-staff meetings
* No ventilator patients; CPAP and BiPAP used on occasion with patients self-managing
* Required skills include Critical Thinking, Chronic Disease Management, Patient/Family Education, NPWT, Ostomy Care, Central Line Care, IV Administration, and Complex Wound Care
* Hospice experience required including pain and symptom management, end-of-life care, empathy, and compassion
* No floating between departments, but will perform both home health and hospice visits
* Solid color scrubs required (no patterns or logos unless company-issued); closed-toe, non-slip shoes with socks or hosiery required
* Charting system: Cerner
* BSN not required
* Required certifications: BLS (AHA)
* Orientation includes 1 day of computer-based training followed by 2-5 days of field training and shadowing
