Job Details:

* Day shift hours from 8:00 AM to 4:30 PM
* Variable units as needed throughout the main campus
* Patient ratio of 25:1
* Utilize Meditech Expanse EMR system
* Careport experience preferred
* Acute care hospital case management experience required
* Hospital discharge assessment and planning experience required, including Home Health, SNF, IRF, wound vac, and oxygen
* Float as needed within the hospital
* Every other weekend or every third weekend required, no on-call responsibilities
* Business casual dress code required, scrubs acceptable
* BSW or MSW certification required
* Excellent communication skills required with ability to thrive in a fast-paced and busy environment

This facility is a regional hospital operating across variable units on its main campus, providing a wide range of acute care services. The hospital maintains a dedicated case management department focused on comprehensive discharge planning and patient transitions to appropriate levels of care.
