Opportunity:
Case Manager
Job Details:
* Minimum 2 years of acute care hospital Case Manager experience required with focus on Care Coordination and Discharge Planning
* Hospital experience is required; behavioral health experience alone does not qualify
* Travel experience required; first-time travelers accepted and will be screened clinically
* Weekend rotation required to meet department needs
* Patient ratio of 1:20
* No floating requirement
* 40 orientation hours required
* Serve patient age groups including adolescents, adults, and geriatrics
* Perform admission criteria assessment using InterQual criteria
* Experience with Care Coordination and Discharge Planning required
* Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, mental health history or crisis, advance directives, POLST, and protective services process required
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Experience with HMOs, IPAs, and similar organizations required
* Must be able to independently arrange DME, SNF placements, and ambulance transport
* Experience with CMS, DRG, Plan of Care, and InterQual criteria required
* Proficiency in Epic charting system required
* Proficient in Microsoft applications required
* Computerized documentation required
* Interdisciplinary support includes Social Services and Transportation
* Scrubs or business attire accepted
* California state license required; pending license accepted but must be active upon start date
* RTO restrictions: no more than 7 days
