Opportunity:

RN - Case Management

Job Details:

* Work Monday through Friday, 8:00 AM to 5:00 PM
* Coordinate management of care and ensure optimum utilization of resources and service delivery
* Conduct comprehensive assessments, care planning, plan implementation, and overall evaluation of individual patient needs
* Enhance quality of patient management and satisfaction to promote continuity of care and cost effectiveness
* Integrate functions of case management, utilization review and management, and discharge planning
* Serve as a patient advocate through resource utilization, discharge planning, and addressing holistic patient needs
* Minimum 2 years of recent RN Case Management experience required, with experience in a LTAC or STACH setting
* Experience with MCG, discharge planning across acute care settings, and strong IDT collaboration required
* Texas RN or multi-state RN license required
* BLS certification through the American Heart Association required
