Physical Therapist - SNF/Home Health

Job Details:

* Cover both skilled nursing facility (SNF) and home health (HH) settings, varying week to week at approximately 50% each
* Primarily based in Grand Island but may be requested to assist in Hastings or Kearney area to cover PTO or obtain full hours
* Supervise 2 therapy assistants
* Software used: Net Health and HCHB
* New graduates welcome - therapy supervisor onsite with clinical specialist and director also available
* SNF productivity expectation: 82% (evaluation, group, and concurrent time counted in full)
* Home health productivity expectation: 95%, which includes drive time and documentation time - 27 points for a full week based on type of visit (80-minute routine visit up to 120-minute evaluation)
* Guaranteed 40 hours per week
* Schedule options: Monday through Friday, Tuesday through Saturday, or Sunday through Thursday - day lengths may slightly vary
* Mileage and drive time paid between locations and home health patients
* No on-call or call-back available
* Care setting: Post Acute
* Required certifications: BLS Certification
