Job Details:

* Day shift position working 5 days per week, 8 hours per day
* Weekend availability required along with 3 weekday shifts per week
* Minimum of 2 years of recent experience in the field required
* Recent hospice experience required
* Provides general nursing care and related services for assigned patients under the direction of a Registered Nurse (RN) or Medical Doctor (MD)
* Performs routine patient visits based on departmental specific policies and procedures
* Assists in the development of prioritized interdisciplinary plans of care for each patient
* Implements and coordinates plans of care utilizing appropriate organizational skills and reporting mechanisms
* Implements individual teaching plans including Patient Teaching Protocols and discharge plans
* Performs assigned clinical procedures including phlebotomy, injections, and drug screens
* Administers medications to assigned patients
* On-call duties may be assigned based on location and agency operational needs
* Required certifications: Licensed Vocational Nurse (LVN) licensure in the state of practice, Basic Life Support (BLS) from an approved vendor
* Arrhythmia certification preferred
* Valid Driver's License required and must be at least 21 years of age or older
