Case Management

Job Details:

* 2 years of case management experience required with acute care hospital experience mandatory and must be current within the last year
* Travel experience required; first-time travelers accepted as a screen with clinical approval
* Required to work every other weekend
* No on-call responsibilities
* Float between units based on departmental need; all travelers expected to float between all facilities within a given state
* Caseload of up to 25 patients
* Patient age groups include adolescents, adults, pediatrics, and geriatrics
* Charting system: Epic (experience required)
* Free parking provided on first day; assignment parking provided during orientation
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* 2 references required covering 1 year within the last 3 years
* Skills checklist must be completed and submitted with application
* Required skills and experience include: HMOs/IPAs, advance directives, POLST, protective services process, discharge planning, care coordination, prior authorizations, appeals and denials, benefits eligibility, DRG, CMS/Medicare/Medicaid, HIPAA guidelines, needs assessment/DME ordering, and interdisciplinary collaboration with social services
