Job Details:

* 12-hour shifts from 7:00 AM to 7:00 PM
* Day shift position
* Every other weekend requirement
* Every other major holiday requirement
* No on-call or standby requirements
* No floating requirements
* No blocked schedule and no self-scheduling
* 2-3 shifts of unit/department specific orientation
* Nurse acts as charge nurse, passes medications, and performs wound care
* Patient ratios: 1:10 on days, 1:19 on nights
* Common diagnoses include heart disease, diabetes, and dementia
* Long term care experience required
* No IV or titratable drips involved
* No telemetry monitoring required
* Oxygen therapy equipment includes concentrators and portable concentrators
* Physician available by phone after hours
* EMR system: MatrixCare/Matrix
* BSN not required
* Required certification: BLS
* Scrubs must be clean and presentable, no specific color requirement

Facility:

This long term care facility is a 26-bed post-acute care setting specializing in the care of residents with chronic conditions such as heart disease, diabetes, and dementia. The facility operates without on-site rehabilitation beds and provides supportive services including physical therapy as needed and a social worker designee. Pharmacy services are managed off-site, and physician support is available by phone after hours.
