Opportunity:
Care Manager
Job Details:
* Day shift schedule with a patient caseload ratio of 1:25
* Weekend requirement is every other weekend, Saturday and Sunday, with a day off during the weekday
* No floating requirement
* Business attire or scrubs required
* 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Experience with HMOs, IPAs, and similar organizations required
* Experience assessing patients, coordinating care, and discharge planning required
* Computerized documentation experience required; Epic charting system experience required
* Must be proficient in care coordination, discharge planning, disease management, and needs assessment/ordering DME
* Knowledge of CMS, CPT coding and billing, DRG, HIPAA guidelines, and Department of Health regulations required
* Experience with admission criteria, medical necessity determination per evidence-based guidelines, and plan of care development required
* Patients served include adults and geriatric populations
* California state license required; pending license accepted but must be licensed upon start date
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* RTO restrictions apply; no more than 7 days allowed
* 2 professional references required covering 1 year within the last 3 years

This is an acute care facility offering a comprehensive range of medical services to adult and geriatric patient populations. The facility supports interdisciplinary care coordination and utilizes Epic as its primary charting system.
