Estimated Gross Weekly Pay: $2799.80

Opportunity:

Care Manager

Job Details:

* Day shift schedule with a patient caseload ratio of 1:25
* Weekend requirement is every other weekend, Saturday and Sunday, with a day off during the weekday
* No floating requirement
* Business attire or scrubs required
* 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Experience with HMOs, IPAs, and similar organizations required
* Experience assessing patients, coordinating care, and discharge planning required
* Computerized documentation experience required; Epic charting system experience required
* Must be proficient in care coordination, discharge planning, disease management, and needs assessment/ordering DME
* Knowledge of CMS, CPT coding and billing, DRG, HIPAA guidelines, and Department of Health regulations required
* Experience with admission criteria, medical necessity determination per evidence-based guidelines, and plan of care development required
* Patients served include adults and geriatric populations
* California state license required; pending license accepted but must be licensed upon start date
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* RTO restrictions apply; no more than 7 days allowed
* 2 professional references required covering 1 year within the last 3 years

This is an acute care facility offering a comprehensive range of medical services to adult and geriatric patient populations. The facility supports interdisciplinary care coordination and utilizes Epic as its primary charting system. The organization maintains high standards for patient care and requires experienced professionals who can work effectively within a fast-paced, collaborative healthcare environment.
