Opportunity:
Pharmacist
Job Details:
* Day shift schedule from 7:30 AM to 6:00 PM with overnight on-call from 6:00 PM to 7:30 AM
* 7 days on / 7 days off schedule aligned with a 2-week pay period
* Overnight on-call coverage for 6 or 7 nights per rotation
* Minimum 1 year of pharmacy experience required
* Inpatient clinical pharmacy experience preferred
* Epic electronic health records experience required
* Current BLS certification required
* Active Pharmacist license in state of practice required
* Doctor of Pharmacy (PharmD) degree preferred
* Process, enter, verify, and distribute medication orders in compliance with department, hospital, and regulatory guidelines
* Monitor and ensure medication therapy is safe, efficacious, and cost-effective
* Review medication prescribing for appropriate indication, dose, route, frequency, allergy history, and drug interactions
* Conduct daily reports on medication utilization to identify areas for improvement
* Participate in clinical rounds and unit-based or population-based medication management activities
* Perform remote order verification and quality assurance procedures
* Collaborate with leadership and pharmacy buyer to procure medications and supplies
* Assist in process improvement recommendations and development of new pharmacy services
* Ensure compliance with drug formulary and appropriate documentation standards
