Opportunity:

RN - Skilled Nursing Unit

Job Details:

* 12-hour day shifts
* Post-acute care setting
* Care for residents with post-surgical conditions and chronic diseases including COPD, CHF, Diabetes, and Renal Failure
* Common admitting diagnoses include hip fractures, pneumonia, sepsis, exacerbation of COPD, CVA, joint replacement, and cancers
* Provide skilled nursing services including wound care, trach care, NG tube, and G-tube management
* Support residents with ADL care and mobility as they progress toward community discharge
* Provide end-of-life care and family education for residents meeting inpatient hospice criteria
* Charting system: EPIC
* BSN not required
* Required certifications: BLS Certification
* Cancellation policy: 3 shifts per 13-week or longer contract; 2 shifts per 12-week or shorter contract

Facility:

This licensed 20-bed skilled nursing facility specializes in short-term skilled care, accepting Medicare A, Medicare Advantage, commercial insurance, and private pay. The facility admits approximately 26 residents and discharges 25 residents monthly, with an average length of stay of 15 days. The unit is equipped to provide comprehensive skilled nursing and therapy services, supporting residents from admission through recovery and discharge back into the community. The facility also maintains agreements with a hospice program, enabling staff to provide compassionate end-of-life care for eligible residents.
