Opportunity:
Physical Therapist - Home Health
Job Details:
* Actively monitor and adjust patient visit schedules to maximize productivity and meet patient needs, including rescheduling visits and communicating changes to supervisor
* Perform comprehensive OASIS and physical therapy assessments of assigned patients, including psychosocial, educational, safety, and risk factor evaluations
* Develop individualized plans of care to promote optimal functional independence and safety, creating patient-specific goals with logical interventions
* Provide skilled physical therapy treatment including therapeutic techniques and patient/caregiver education, assessing patient status and progress on each visit
* Coordinate patient care with other clinical team members including physicians, nurses, caregivers, and rehabilitation services as Care Manager and/or team member
* Supervise and direct clinical activities of PTAs, PT students, and home care aides in accordance with federal and state regulations and APTA guidelines
* Complete and coordinate required clinical documentation to accurately reflect care provided and outcomes achieved
* Ensure accurate charging and billing processes including insurance coverage parameters and required insurance forms
* Participate in in-service training programs for rehabilitation, medical, nursing staff, volunteers, and community groups
* Carry out patient-related health assessments including PT/INR and blood pressure monitoring
* Perform patient-related procedures including staple/suture removal and wound care
* Complete OASIS time points in accordance with agency, state, and federal guidelines
* Collaborate with quality improvement staff to maximize accuracy of clinical notes and OASIS documents
* Accurately complete agency, state, and federal forms including HHABN and Medicare Notice of Non-Coverage in a timely manner
* Assess patients' needs for home care aide services and develop and coordinate plans of care with patients and caregivers as necessary
* Coordinate safe patient discharges and/or transfers and ensure all appropriate paperwork is processed in accordance with department policies and regulations
* Participate in regular peer review audits and provide constructive professional feedback
