Physical Therapist

Job Details:

* Day shift schedule, Monday through Friday
* 4 x 9-hour shifts and 1 half day totaling 40 hours per week
* Monday through Thursday 8:00 AM to 5:30 PM and Friday 8:00 AM to 12:00 PM
* No on-call responsibilities
* Doctor of Physical Therapy (DPT) degree required if license obtained on or after January 2016 or foreign equivalent
* Minimum 2 years of Physical Therapy experience preferred, 3 years preferred
* Physical Therapy licensure in state of practice required
* Basic Life Support (BLS) certification from approved vendor required
* Prior EPIC experience is a bonus
* Complete evaluations to ensure quality standards and address assessment areas requiring attention
* Use appropriate clinical techniques and tools to establish individualized treatment plans for patients
* Provide therapy interventions using appropriate treatment protocols and techniques
* Establish individualized care and discharge plans and ensure plans are communicated to appropriate parties
* Complete comprehensive and accurate clinical documentation within established deadlines
* Supervise assigned assistants
* Serve as co-leader in interdisciplinary approach to patient groups
