Opportunity:

Home Health PT - Mt. Angel, OR and Surrounding Areas

Job Details:

* 8-hour day shifts (facility cannot accommodate 4x10s or 3x12s)
* Visit up to 6 patients per day with 30 productivity points standard (1:1 ratio)
* Northern Territory coverage area includes Woodburn, Mt. Angel, Aurora, Molalla, Gervais, and Silverton
* No weekend requirements
* No on-call requirements
* Facility may call off up to one shift per pay period (every two weeks)
* Minimum 1 year of skilled home health experience required
* First-time travelers accepted
* Required certification: BLS/BCLS
* Epic charting experience preferred; charting completed via EPIC Remote Client on laptop and Rover phone app
* OASIS documentation required
* Traveler must have their own reliable transportation
* Occasional maximum travel radius of approximately 60 miles per day (60-90 minute total drive time)
* System groups clients to minimize drive and windshield time
* Attire: Business casual or scrubs (no jeans, no specific scrub color required)
* Laptop and work cell phone provided for documentation and work communication
* Mileage reimbursement provided at IRS standard rate
* Free parking
* Skills in geriatrics, home health, OASIS, and orthopedics preferred
* Patients served include adults and geriatrics
* Interpretation services available as interdisciplinary support
