Opportunity:

Home Health PT - Medford OR and Surrounding Areas

Job Details:

* 8-hour day shifts
* 1:1 patient ratio with visits of up to 6 patients per day; 30 productivity points standard
* No on-call requirements
* No weekend requirements
* Guaranteed hours with facility able to call off up to one shift per pay period (every two weeks)
* Minimum 1 year of skilled home health experience required
* Oasis experience required
* BLS certification required
* Traveler must have their own reliable transportation
* Occasional travel radius of up to 50+ miles; system groups clients to minimize drive and windshield time
* Charting system: EPIC/Oasis
* Attire: Any color scrubs
* Mileage reimbursement provided at IRS standard rate
* Free parking available
* Skills expected include: acute care, outpatient clinic, rehabilitation, skilled nursing, body mechanics, orthopedic conditions, neurological conditions, post-stroke rehabilitation, spinal cord injury, traumatic brain injury, home exercise programs, and patient/family education
