Job Details:

* Consult-based position with a varying caseload
* Weekend rotation required on an every other weekend schedule
* Float as needed to meet department needs
* Minimum 2 years of acute hospital Social Work experience required
* Travel experience required
* Must be first-time traveler eligible (screening required)
* Charting system experience required; Epic charting system
* MSW certification required
* Conduct biopsychosocial assessments and initial evaluations
* Perform care coordination and assist with care conferences and family meetings
* Participate in the development and revision of patient treatment plans
* Knowledge of drug and alcohol abuse treatment options required
* Experience working with patients experiencing homelessness, mental health history, or in crisis required
* Knowledge of POLST, Advance Directives, and NASW Code of Ethics required
* Experience with 5150/involuntary holds required
* Serve patient age groups including neonates, infants, adolescents, pediatrics, adults, and geriatrics
* Coordinate with internal and external team members to ensure continuity of care
* May be expected to float between sister facilities nearby contracted facility
