Opportunity:

LPN/RN - SNF/LTC

Job Details:

* 12-hour day shifts from 6:45 AM to 7:15 PM
* Every other weekend required
* No on-call or standby requirements
* No self-scheduling; scheduled according to facility needs
* 2 shift orientation period
* Cancellation policy: 2 shifts per 13-week contract
* Float as needed; travelers will float before core staff
* Nurse-to-patient ratio: RN/LPN 1:15; CNA 1:10
* RN responsibilities include IV management, PICC lines, mediports, and central lines including IV antibiotics, blood, and IV fluids
* LPN responsibilities include medication administration, wound care, foley/catheter care, PEG tube feedings, ostomy care, and tracheostomy care
* Common patient diagnoses include sepsis, femur fractures, NSTEMI, CKD, CHF, and UTI
* Charting system: Point Click Care (unit notes) and EPIC
* Required certifications: BLS/CPR certification must be obtained prior to first day
* Navy blue scrubs required for RN and LPN positions
* BSN not required

Facility:

This post-acute care facility is a skilled nursing and long-term care facility housing 60 residents across two units of 30 residents each, all in private rooms. The facility maintains a comprehensive interdisciplinary team including physical therapy, occupational therapy, speech therapy, social services, case managers, dieticians, a pharmacist, and medical records staff all on-site. Physicians are available to be called 24/7, and there is always a charge nurse and RN present in the facility.
