Opportunity:

Care Manager

Job Details:

* Minimum 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Patient caseload of up to 25 patients on days
* Weekend requirement is every other weekend, Saturday and Sunday, with a compensatory day off during the weekday
* No floating required
* First-time travelers accepted but must be screened
* California state license required; pending license accepted but must be active upon start date
* Travel experience required
* Experience with HMOs, IPAs, and similar organizations preferred
* Experience assessing patients, performing care coordination, and discharge planning required
* Computerized documentation experience required; Epic charting system experience required
* Experience with CMS, CPT coding and billing, DRG, HIPAA guidelines, and determining medical necessity per evidence-based guidelines
* Experience working with adult and geriatric patient populations
* Skills required include: Acute Hospital, Admission Criteria, Care Coordination, Disease Management, Discharge Planning, ICU, MS, Needs Assessment/Order DME, Plan of Care, and SDU/PCU/IMC/Obs
* RTO restrictions: no more than 7 days
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* Dress code: Business attire or scrubs
