Opportunity:

RN - Case Manager

Job Details:

* Minimum 3 years of acute care case management experience required
* Utilization management experience required
* Active California RN license required - no pending or temporary licenses accepted
* Required certification: BLS
* Epic charting system experience highly preferred
* No weekend shifts required
* No floating required
* No on-call required
* Patient ratio of 15 patients per day
* Serves adolescent, adult, and geriatric patient populations
* Proficiency in discharge planning, care coordination, and plan of care development
* Experience with InterQual criteria and Milliman guidelines preferred
* Knowledge of ICD-10 coding, DRG, CPT coding, and CMS guidelines preferred
* No scheduling requests can be accommodated - must be fully flexible
* Interdisciplinary support includes social services and transportation
* Flu vaccination required - no exemptions
