Opportunity:

RN - Case Manager

Job Details:

* Minimum 2 years of acute care case management experience required
* California case manager experience required (minimum 6 months acceptable)
* Active California RN license required at time of submission - no pending or temporary licenses accepted
* Required certification: BLS
* Epic charting system highly preferred
* No weekends required
* No floating required
* No on-call required
* No scheduling requests can be accommodated - must be flexible
* Patient ratio: 15 patients per day
* Treats adolescent, adult, and geriatric patient populations
* Skills required include discharge planning, care coordination, plan of care, InterQual criteria, Milliman guidelines, ICD-10 coding, and HIPAA guidelines
* Flu vaccination required with no exemptions
* 2 supervisor or higher references from within the past year required at time of submission

This well-established healthcare facility operates as a large acute care hospital with a daily census of approximately 255 patients. The facility provides interdisciplinary support services including social services and transportation, and maintains a collaborative care environment.
