Job Details:

* Work Monday through Friday from 8:00 AM to 5:00 PM
* Some weekend coverage is required
* Some on-call may be scheduled on an emergency basis only
* Travel from patient to patient for home visits
* Must be able to manage and adjust to changes in schedule
* OASIS (Outcome and Assessment Information Set) experience required
* Home health and hospice experience required
* Must have a valid driver's license in good standing
* BLS certification required
* EMR system used is HealthWyse (Careficient)
* Outpatient care setting
