Opportunity:

RN - GI Navigator (Ambulatory Clinic)

Job Details:

* Day shift hours from 8:00 AM to 5:00 PM
* Minimum 1 year of experience required
* Medical/surgical experience required; ambulatory experience is a plus and highly preferred
* Must have experience with MS (Multiple Sclerosis) patient care
* Work with adult and geriatric patient populations (18 years and above)
* Clinic setting with one clinic day per week seeing patients in person
* Manage 3-4 rooms per MD and PA
* Handle 10-30 phone messages per day and manage emails through MyChart
* Follow up on lab results from off-site labs including Quest and LabCorp
* Work in the same workspace as the designated MD
* Assist with patient discharge education, medication instructions, and follow-up coordination
* Administer B12 injections and assist with infusions
* Coordinate direct admissions and manage authorization processes as RN Navigator
* Assist with biopsy follow-up and care coordination prioritization
* Work with patients diagnosed with general GI conditions, Crohn's Disease, and Inflammatory Bowel Disease
* Float within the same clinic to other navigator roles as needed
* EPIC experience required and must be noted in work history
* Required certifications: BLS, CCRN (must be in-hand when applying)
* Valid California state license required in-hand when applying (EMSA not accepted)
* Two supervisor references required when applying
* Skills required: adult patient care, intramuscular injections, intravenous infusions, patient teaching, pharmacy calls and patient refill requests, subcutaneous injections, telephone triage, and minor injections including B12
