Job Details:

* Cover both Skilled Nursing Facility (SNF) and Home Health (HH) settings, varying week to week at approximately 50% each
* Primarily based in Grand Island but may be requested to assist in Hastings or Kearney area to cover PTO or obtain full hours
* Supervise 2 assistants
* Utilize Net Health and HCHB software systems
* SNF productivity expectation: 82% (evaluation, group, and concurrent time counted in full)
* Home Health productivity expectation: 95%, which includes drive time and documentation time; 27 points for a full week based on type of visit (80-minute routine visit up to 120-minute evaluation)
* Guaranteed 40 hours per week
* Schedule options: Monday through Friday, Tuesday through Saturday, or Sunday through Thursday; day lengths may slightly vary
* Mileage and drive time paid between locations and home health visits, and from patient to patient in home health
* Care setting: Home Health
* Required certifications: BLS Certification
* No on-call or call back available
