Opportunity:

LVN - Skilled Nursing

Job Details:

* Assess, plan, and implement nursing care of residents within a skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing
* Administer medications following the eight (8) medication rights to reduce potential for medication errors
* Perform waived testing (point-of-care testing) and interpret results per clinical policies and procedures
* Document changes in residents' conditions and nursing care on progress notes and care plans
* Notify physician of changes in resident condition and follow through until appropriate action is taken
* Conduct physician orders in a timely manner and participate in walking rounds for shift reporting
* Implement plan of care based on assessments and goals established by the interdisciplinary care team
* Supervise CNAs and supportive staff members to ensure continuity of care between shifts
* Function as resident advocate by protecting residents' rights and addressing psychosocial, cultural, ethnic, and religious/spiritual needs
* Participate in resident and family teaching including pain management education
* Demonstrate competence in technical and manual skills according to LVN scope of practice
* Follow proper infection control procedures and supervise supportive staff in infection control
* Participate in orientation of new personnel and assist in performance evaluations when needed
* Graduate of an accredited LVN program required
* Current California Licensed Vocational Nurse license required
* Current BLS certification required
* Basic computer knowledge required; Electronic Health Record experience preferred
* Ability to read and communicate effectively in English required; additional languages preferred
