Home Health PTA

Job Details:

* 8-hour day shifts
* No weekend requirement
* No on-call requirement
* Visit up to 6 patients per day with 30 productivity points standard (1:1 ratio)
* Facility can call off up to one shift per pay period (every two weeks)
* Serve patients in East Portland, OR / Clark County, WA and surrounding areas
* Travel required - provider must have own transportation
* System groups clients to minimize drive time; occasional maximum radius of roughly 60 miles (60-90 minute total drive time) per day
* Both Oregon and Washington licensure required
* Minimum 1 year of skilled home health experience required
* First-time travelers accepted
* Local candidates accepted within 50-75 mile radius
* Pending license accepted
* Charting via EPIC Remote Client for Home Health (on laptop) and Rover phone app/Oasis
* Business casual attire required; no jeans; scrubs allowed but not required (no specific color)
* Mileage reimbursement at IRS Standard rate
* Required certification: BLS
* Skills required include: acute care, outpatient clinic, rehabilitation, skilled nursing, use of assistive devices, orthopedic conditions (hip fractures, total hip/knee replacement, post-op surgeries), neurological conditions (MS, Parkinson's, post-stroke rehabilitation, TBI, spinal cord injury), and cognitive disorders
