Job Details:
* Work 8-hour shifts Wednesday through Saturday from 4:00 PM to 2:30 AM
* Weekend requirement every Saturday
* Holiday rotation with staff required
* First to float as needed throughout the hospital
* Primary placement in the Emergency Department with the ability to float in-house based on hospital need
* See 5-8 patients/consults per day
* White lab coat required
* EMR system: Cerner
* Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients
* Conduct comprehensive assessments and develop treatment plans integrating medical, social, and resource issues
* Provide crisis intervention, patient/family intervention, high-risk screening, and brief counseling
* Assist with post-acute placement on complex discharges and engagement of appropriate community resources
* Collaborate with physicians, case managers, nursing staff, county agencies, and community resources
* Arrange and facilitate family/patient representative meetings with the healthcare team as needed
* Serve on hospital committees and participate in developing and presenting in-services to hospital staff
* Master's degree from a Council on Social Work Education accredited school of Social Work required
* Minimum 2 years of clinical and/or medical social work experience required
* Must obtain 5150 certification at the county level
* Required certifications: BLS, MSW, 5150
* Must have prior recent CA Dignity experience to be considered
* Strong interpersonal, written communication, and customer service skills required
* Demonstrated ability to manage multiple tasks, work independently, and thrive in a fast-paced environment
