Job Details:

* Monday through Friday schedule from 8:00 AM to 5:00 PM with holiday coverage as needed
* Outpatient care setting with travel required between patient homes for home visits
* Must have OASIS experience
* Must have home health and hospice experience
* Must have experience traveling from patient to patient for home visits
* Must be able to manage and adjust to changes in schedule
* EMR system used is HealthWyse
