Case Management

Job Details:

* Minimum 2 years of case management experience required; acute care hospital experience must be current within the last year
* Travel experience required; first-time travelers accepted with clinical approval
* Every other weekend coverage required
* No on-call responsibilities
* May float between units based on departmental need
* Caseload of up to 25 patients
* Charting system: Epic (experience required)
* Free parking provided on first day; assignment parking provided during orientation
* Patient population includes adolescents, adults, pediatrics, and geriatrics
* Required skills and experience include: HMOs/IPAs, advance directives, POLST, protective services process, discharge planning, care coordination, benefits eligibility, appeals and denials, prior authorizations, DRG, CMS/Medicare/Medicaid, HIPAA guidelines, needs assessment/DME ordering, and workers' compensation case management
* Interdisciplinary support available through social services
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* All RTO and specific scheduling requests must be submitted upfront at time of submittal
* Compact license must be in hand at time of submission
* 2 professional references required, covering 1 year within the last 3 years
