Opportunity:

RN - Home Health/Hospice

Job Details:

* ASAP start needed
* Work 5 eight-hour days, Monday through Friday, with weekend and on-call rotation during business hours 8:00 AM to 5:00 PM
* Weekend rotation typically occurs once or twice a month, varying depending on staffing
* On-call requirement is overnight from 5:00 PM to 8:00 AM, typically once or twice a month but could be more depending on staffing
* Holiday coverage required if assignment aligns with major holidays (New Year's Day, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas)
* Manage a caseload averaging 15 to 20 patients
* Care for patients with oncology diagnoses, Dementia/Alzheimer's, end stage CHF/COPD, and end stage neurological conditions (ALS, MS, Parkinson's)
* Drive independently from patient home to patient home using own vehicle - valid driver's license and personal vehicle required
* May be asked to support other agency locations if census is low
* 1 to 2 weeks of orientation provided
* EMR system: Netsmart
* IV pumps provided through vendor
* Dress code: Business casual or scrubs, no specific color requirement
* Required certifications: BLS
* Maine RN license required
* Minimum 1 year of experience required; first-time travelers accepted only with hospice experience

Facility:

This home health and hospice organization serves a census averaging between 85 to 90 patients in the South Portland area. The facility provides comprehensive support to its clinical staff through a dedicated scheduling team, clinical manager, and lead support team. Travelers are supported with a structured orientation process and have access to necessary clinical resources and supplies at the office location.
