Opportunity:
RN - Clinical Care Registered Nurse (CCRN)
Job Details:
* Provide direct nursing care to participants, working independently with advanced nursing knowledge and skills
* Triage emergent situations and manage complex clinical situations independently
* Monitor acute and chronic medical conditions in coordination with Primary Care Providers
* Utilize care plans independently to provide clinic and home-based direct care
* Administer treatments, procedures, and medications as prescribed
* Conduct regular home visits to provide care and treatment including wound care, lab draws, nurse assessments, and other nursing tasks
* Work beyond normal working hours and on weekends and holidays as necessary
* Collaborate with both licensed and non-licensed personnel
* Bachelor's degree in Science of Nursing preferred; Associate's degree required
* RN Nursing Diploma or Degree from an accredited School of Nursing or University required
* Minimum of one year experience working with frail, elderly, or long-term care population required
* Minimum of one year home care experience preferred
* Minimum of one year wound care experience preferred
* Current CPR certification by AHA required or must be willing to obtain
* Must have reliable transportation, current valid Michigan driver's license, and car insurance
* Must be 18 years of age or older
