Opportunity:
RN - Home Health/Hospice
Job Details:
* ASAP start needed with manager interviews required
* 8-hour day shifts, Monday through Friday, with business hours from 8:00 AM to 5:00 PM
* Weekend rotation required, typically once or twice a month depending on staffing
* On-call rotation overnight from 5:00 PM to 8:00 AM, typically once or twice a month but may vary based on staffing needs
* Holiday coverage required if assignment aligns with major holidays including New Year's Day, Memorial Day, July 4th, Labor Day, Thanksgiving, and Christmas
* Case manager averages 15 to 20 patients per caseload
* South Portland office census averages between 85 to 90 patients
* Care for patients with oncology diagnoses, Dementia/Alzheimer's, end stage CHF/COPD, and end stage neurological conditions including ALS, MS, and Parkinson's
* Must use own vehicle to drive from patient home to patient home
* May be asked to support other agency locations if census is low
* 1 to 2 weeks of orientation provided
* EMR system: Netsmart
* IV pumps provided through vendor
* Business casual attire or scrubs required with no specific color requirement
* Minimum 1 year of experience required; first-time travelers accepted only with prior hospice experience
* Required certifications: BLS
* Maine RN license required
* Valid driver's license required
