Opportunity:

RN - Hospice Home Care

Job Details:

* Full-time position, Monday through Friday with a weekend rotation
* Shift times are 5 (8-hour) days per week, business hours 8:00 AM to 5:00 PM; 4 (10-hour) days may be considered dependent upon experience
* Will work between two locations: Bangor and Ellsworth
* Recent experience providing Hospice Care in a home setting required
* Case Manager averages 25 to 30 patients; Ellsworth office census averages 150 patients
* Patient types include post-orthopedic surgeries, wound care, chronic/acute CHF and COPD, post-cardiac surgeries, genitourinary, IV antibiotic teaching/central line care, medication pre-fills, and lab draws
* Weekend rotation typically occurs once a month, but may be more frequent depending on staffing
* Overnight on-call rotation from 5:00 PM to 8:00 AM, typically once every 3 to 4 weeks
* Holiday coverage required if assignment aligns with major holidays (New Year's Day, Memorial Day, July 4th, Labor Day, Thanksgiving, Christmas)
* May be asked to support other agency locations if Ellsworth territory census is low
* EMR system: Netsmart; IV pumps provided through vendor
* Dress code: Business casual or scrubs; no specific color requirement
* Must hold a valid driver's license and use own vehicle to travel between patient homes
* Required certifications: BLS
* Must be licensed as an RN in the State of Maine
* Minimum 1 year of experience accepted; first-time travelers accepted only with Home Health and Hospice experience
* 1-week orientation provided

Clinicians are supported by a scheduling team, clinical manager, and a lead support team to ensure quality patient care is delivered efficiently across the service area.
