RN - Home Health/Hospice

Job Details:

* Work Monday through Friday from 8:00 AM to 5:00 PM with on-call rotation for evenings and weekends
* 24-hour on-call coverage required on weekends and holidays
* Holiday coverage required
* BLS certification required
* Home Health & Hospice experience required
* Must have a current Driver's License and Auto Insurance
* Any solid color scrubs required
* Use HomeCare HomeBase EMR system
* Conduct initial assessments of clients and families to determine home health needs including complete physical assessments and history of current and previous illness
* Regularly re-evaluate client nursing needs and use health assessment data to determine nursing diagnosis
* Initiate and revise plan of care as client status and needs change
* Develop care plans and establish goals based on diagnosis incorporating therapeutic, preventive, and rehabilitative nursing actions
* Administer medications and treatments as prescribed by physician
* Provide health care instructions to clients as appropriate per assessment and plan of care
* Identify discharge planning needs and implement prior to discharge
* Act as Case Manager when assigned, coordinating client care for assigned caseload
* Prepare clinical notes and update primary physician when necessary and at least every sixty days
* Communicate with physicians regarding client needs and report any changes in client condition
* Ensure arrangements for equipment and other necessary items and services are available
* Instruct, supervise, and evaluate home health aide care provided
* Serve patients within a 60-mile radius including homes, nursing homes, and other care settings
* Treat adult, geriatric, and pediatric patient populations
