Opportunity:

LVN - Skilled Nursing

Job Details:

* Must have skilled nursing experience
* Assess, plan, and implement nursing care of residents within the skilled nursing setting under the supervision of a Registered Nurse and Director of Nursing
* Administer medications following the eight (8) medication rights to reduce the potential for medication errors
* Ensure continuity of care between shifts by providing direct care and supervising CNAs and supportive staff members
* Perform waived testing (point-of-care testing) and interpret results, taking appropriate action as needed
* Document changes in residents' conditions and nursing care on progress notes and care plans
* Notify physician of changes in resident's condition and follow through until appropriate action is taken
* Implement plan of care based on assessments and goals established by the interdisciplinary care team
* Supervise and delegate tasks to CNAs and other healthcare team members to accomplish the plan of care
* Function as a resident advocate by protecting residents' rights and addressing psychosocial, cultural, ethnic, and religious/spiritual needs
* Participate in orientation of new personnel and assist in performance evaluations when needed
* Follow proper infection control procedures and supervise supportive staff in infection control procedures
* Must be a graduate of an accredited LVN program
* Current California Licensed Vocational Nurse license required
* Current BLS certification required
* Electronic Health Record experience preferred
* Basic computer knowledge required
* Ability to read and communicate effectively in English required; additional languages preferred
