Opportunity:

PT - Physical Therapist

Job Details:

* Day shift, 8:00 AM to 5:00 PM
* Full-time position
* Provide physical therapy services to referred patients including assessment, treatment plan development, implementation, follow-up, and discharge planning
* Perform comprehensive diagnostic evaluations to determine physical and cognitive abilities of inpatients and outpatients based on diagnosis and age-specific needs
* Develop and implement individualized treatment plans with patient and family participation, including measurable goals and methods to achieve goals
* Adhere to documentation policies and procedures including initial patient evaluations, daily and weekly progress notes, discharge summaries, and other appropriate notations
* Participate in patient care conferences, family conferences, in-service programs, and departmental and interdepartmental meetings
* Counsel patients, families, and other involved individuals regarding patient evaluation, care, and treatment
* Initiate and complete discharge planning in a timely manner and provide effective education and training
* Assist with coordination and supervision of Physical Therapy and Physical Therapy Assistant student programs as well as the volunteer program
* Assist with clinical aspects of the department including orientation of new associates and scheduling of patients
* Support and participate in Performance Improvement activities
* Maintain awareness of community agencies and resources to make appropriate referrals when indicated
* Supervise Licensed Physical Therapy Assistants, Rehab Technicians, and Volunteers
* Bachelor's, Master's, or entry-level Doctorate of Physical Therapy degree from an accredited college or university required
* Current New Mexico Physical Therapist Licensure required
* BLS certification via AHA required
* Proficiency with all modalities and equipment used by a Physical Therapist as outlined by the Executive Council of Physical Therapy and Occupational Therapy Examiners
