* Complete resident assessment tools in accordance with all Federal and State regulations
* Develop and maintain resident care plans according to facility policy and procedure
* Facilitate interdisciplinary team meetings to support individualized nursing care
* Ensure all documentation and assessments meet regulatory compliance standards for skilled nursing facility services
* Provide optimum individualized nursing care coordination for SNF residents
