Opportunity:

NP - Hospice and Palliative Care

Job Details:

* Monday through Friday schedule with one Sunday per month required, with a weekday off in exchange
* Shift hours from 8:00 AM to 5:00 PM
* Home-based position requiring local travel for patient visits with mileage reimbursement at 70 cents per mile
* Valid California driver's license and insurance required
* Conduct 45 to 50 home visits per month providing face-to-face hospice visits
* Perform patient assessments, develop care plans, and manage symptoms for patients with serious or life-limiting illnesses
* Conduct goals-of-care discussions, advance care planning including POLST and advance directives, and patient and family education
* Coordinate care with the interdisciplinary team, home aides, and community physicians
* Document all patient encounters in My Unity EMR and participate in IDT meetings and quality initiatives
* Participate in rotating weekends, occasional on-call coverage, and 24/7 patient coverage when needed
* Required credentials: California NP and RN licenses, DEA, NPI, MSN, AANP/ANCC certification, and BLS
* Minimum 3 years of hospice or palliative care experience required
* ACHPN certification required within one year of hire, preferred at time of hire
* Spanish bilingual skills are a plus
