Opportunity:
Case Management
Job Details:
* Day shift position with a patient ratio of 1:20
* Case load of up to 25 patients
* Every other weekend required with availability for holidays including Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day
* No floating required
* No on-call required
* 40 orientation hours required
* Minimum 2 years of acute care case management and discharge planning experience required
* Travel experience required; first-time travelers will be screened
* California state license required; pending license accepted but must be active upon start date
* Epic charting system experience required
* Dress code: Business attire or scrubs
* Perform admission criteria reviews including InterQual guidelines for ED, Mother Baby, and NICU
* Experience with care coordination, assessment of patients, and discharge planning required
* Knowledge of HMOs, IPAs, Medicare, and MediCal required
* Competent with arranging DME, SNF, and ambulance transport independently
* Knowledge of adoption/foster care, drug/alcohol abuse treatment options, homelessness resources, mental health history, advance directives, POLST, and protective services process required
* Ability to organize and participate in family meetings and contribute to treatment plan development
* Will be required to monitor up to 2 units on weekends with a focus on discharge planning
* Knowledgeable in Medicare guidelines required
* InterQual experience highly preferred
* Acute behavioral health experience does not meet acute hospital requirements
* Computerized documentation required
* Serves patient age groups including neonates, infants, adolescents, pediatrics, adults, and geriatrics
* Interdisciplinary support available including IV teams, physical therapy, respiratory services, pharmacy, social services, and more
* RTO restrictions: No more than 7 days
