Opportunity:
RN - Case Management
Job Details:
* Day shift, Monday through Friday, 40 hours per week
* Onsite position
* Current RN licensure required
* Current BLS certification required
* At least two years of recent experience in the field required
* 5-10 years of acute and ancillary experience in multiple modalities preferred
* Case Management certification (CCM) preferred
* Bachelor's Degree in Nursing (BSN) preferred
* Manage development, implementation, and evaluation of care management service lines
* Partner with physician advisors, claims department, and data management teams to support member population outcomes
* Perform and/or coordinate preventative care, wellness, and nutritional counseling for members
* Collaborate with physicians and staff to ensure medical necessity criteria are met
* Manage discharge planning and utilization management communications
* Work on quality and performance improvement activities related to optimal utilization of resources and efficient delivery of care
* Preferred experience working in a health plan environment focused on utilization management and case management activities
* Strong communication skills required with experience coaching patients and members through their healthcare journey
