Opportunity:

Care Manager

Job Details:

* Minimum 2 years of acute medical Care Manager experience performing Care Coordination and Discharge Planning required
* Patient caseload of up to 25 patients (1:25 ratio)
* Weekend coverage required every other weekend, Saturday and Sunday, with a compensatory day off during the weekday
* No floating required
* Patients served include adults and geriatric populations
* Experience with HMOs, IPAs, and similar organizations required
* Experience with assessing patients, care coordination, and discharge planning required
* Computerized documentation experience required; Epic charting system experience required
* Skills required include: acute hospital care, admission criteria, care coordination, CMS knowledge, CPT coding and billing, Department of Health regulations, determining medical necessity per evidence-based guidelines, discharge planning, disease management, DRG knowledge, HIPAA guidelines, ICU, MS, needs assessment/ordering DME, plan of care, and SDU/PCU/IMC/Obs experience
* Business attire or scrubs acceptable
* Travel experience required
* First-time travelers accepted but must be screened prior to placement
* California state license required; pending license accepted but must be active upon start date
* RTO restrictions: no more than 7 days
* Must commit to working 3 of the 5 holidays: Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, and New Year's Day

This is an acute care facility providing comprehensive medical services to adult and geriatric patient populations. The facility utilizes Epic as its primary charting system and supports a multidisciplinary care environment focused on care coordination, discharge planning, and patient management across various levels of care including ICU, SDU, PCU, and observation units.
